Today is June 12, 1998
You are a resident, at the Hertel Elmwood Ambulatory site. You are the
Primary Care Physician for Elaine Stuart. This is your sixth encounter
with this patient. As a resident, you are supervised by Dr. Freer.
- This follow up visit occurs about three weeks after the last office
visit (5/15/98).
- Ms. Stuart has been experiencing progressive symptoms since the last
visit.
- She enrolled in Hospice on June 8, 1998.
- Discuss with the patient her current symptoms and concerns at this
point in time and perform an appropriate physical exam.
- Write a note that develops your plan for the care of this patient.
