Today is May 8, 1998
You are a resident, at the Hertel Elmwood Ambulatory site, seeing this
patient for the fifth time. You are the Primary Care Physician for Elaine
Stuart. As a resident, you are supervised by Dr. Freer.
- This follow up visit occurs a week after the last office visit. Ms.
Stuart also saw her oncologist, Dr. Johnson, on May 11th; he recommended
more aggressive chemotherapy. Ms. Stuart met with someone at Hospice on
May 13th.
- Discuss with the patient her symptoms and concerns at this point in
time.
- Write a note that develops your plan for the care of this patient.
