Student/Resident Instructions
Office Visit 2
Today is January 10, 1998
You are a resident, at the Hertel Elmwood Ambulatory site, seeing this
patient for the first time. You are the Primary Care Physician for
Elaine Stuart. As a resident, you are supervised by Dr. Freer.
- You are to see this patient and do an appropriate history and
physical examination.
- Discuss the need for a Health Care Proxy and Advance
Directives.
- Write a note that develops your plan for the care of this patient.
The patient has been seen in the practice before and has called for an
appointment because she has observed blood in her stool.
(Please note you are to interview the patient, make an appropriate
transition to the physical exam, exit the room and allow the patient time
to change, re-enter the room and complete the physical examination, exit
the room and allow the patient time to change, and re-enter the room to
discuss your physical findings and the need for further tests and/or
procedures.)
