Initial presentation with complaints of blood in stool (which have been
going on for one month prior to this visit).
Visit #3: 1/24/98

Office visit to discuss details of scope biopsy (which was done on 1/14/98
& was + for cancer), and to make arrangements for surgery.
Visit #4: 2/5/98
Time: 3.5 wks
Hospital visit (after surgery on 2/2/98) to discuss surgical findings
(Stage III Ca) and to discuss referral to oncologist for adjuvant chemo
(which will start on 2/23/98)
Visit #5: 5/8/98
Time: 4 mo
Office visit 3 mo after surgery (2+ mo after starting adjuvant chemo), and
a few days after last onc visit (5/4/98) at which time symptoms prompted
onc to order CT & lab (done 5/5/98). This visit will lay out options (more
chemo as per onc) or hospice/pall care.
Visit #6: 5/15/98
Time: 4+ mo
Follow-up visit after onc (5/11/98) and hospice (5/13/98) to discuss
options in greater detail. Pt decides on hospice. Symptoms addressed &
treated.
Visit #7: 6/12/98
Time: 5+ mo
One mo later, enrolled in hospice and having progressive symptoms.
